Although the number of studies and knowledge about the appropriate diagnosis and rehabilitation of hamstring injuries is increasing, the incidence of these injuries in football remains quite high. Today, we know - and this is very important information - that not all hamstring injuries heal at the same rate and they are not the same; some require longer rehabilitation and a longer absence from the field, especially if they involve proximal tendons. It is crucial to conduct an adequate diagnosis from the start to obtain the best and most accurate picture of the type of hamstring injury, that is, whether it is indeed a hamstring injury, a proximal or distal tendon injury, or if it is referred pain from another region or structure. Through a good anamnesis combined with a clinical examination and radiological diagnostics, we get a clear picture of the exact injury, which is of utmost importance as the progression of rehabilitation greatly varies depending on whether it is an a, b, or c type of hamstring injury. There are various passive modalities that can be used in therapy; however, a quality and individualized progression of kinesiotherapy interventions, accompanied by objective assessment and testing, represents the gold standard in the rehabilitation of hamstring injuries, regardless of whether it involves the muscle or the tendon. Literature is quite limited regarding the treatment and rehabilitation protocols following a proximal hamstring tendon injury, and surgical treatment is often recommended. In rehabilitation, due to limited evidence, the experience of the therapist and all those involved in the recovery process is crucial, as is understanding and respecting the healing times of individual structures to properly manage the progression using available algorithms and protocols for treating hamstring ruptures and return to play.
